Are the President and Congress cooking up a plan to dump helpless seniors and disabled persons into “medical homes” in order to reduce costs to Medicare and HMOs?
That’s the worry of some private citizens and healthcare professionals including the Center for Medicine in the Public Interest (CMPI).
Buried in the Obama health care reform bill is a proposal to establish a “medical home pilot program” to care for elderly and disabled citizens. As pitched by HHS Secretary Kathleen Sebelius, the plan would provide a medical home for every citizen while the country transitions from a sickness-based to a wellness-based health system.1
While it may sound compassionate at first glance, the medical home would be run by nurse practitioners and physician assistants who would dole out treatment to residents without the supervision of a physician. Instead, they would follow the guidelines of EBM (evidence-based medicine) — a formulaic approach that is based on statistical and clinical research.
According to CMPI, while EBM is a valuable tool in the hands of an experienced practitioner, it is now being “distorted by government bureaucrats and HMOs in ways that impose top-down, one-size-fits-all restrictions on patients and their healthcare providers.” 2 In other words, cookie-cutter medicine will be practiced by partially trained medical personnel in order to save the government and private insurance companies money.
American Life League’s Judie Brown is even more blunt when she says, “… the nationalized health-care system appears more and more like a behind-the-scenes project based on government control over who lives and who dies.”3
There may be some shocking truth to this when you consider the fact that the pilot program will be targeting Medicare recipients with a high medical “risk score” or who require continual monitoring or treatment. Does this sound like the type of person whose sole medical treatment should be coming from a nurse?
Along with the lack of physician care for individuals that may need it most, I have some serious reservations about the government implementing and overseeing this huge healthcare undertaking. There are currently 22 million people that are eligible for the medical home program3 — the combined populations of Michigan and Pennsylvania!
Can the government really implement a workable program to care for this staggering number of people? What kind of oversight will there be and who will take the ultimate responsibility for their well-being? Will we simply be adding jobs to the already bloated government payroll or is there real cost-savings in this plan? Our current health care system needs a major overhaul, but is this too much of a risk to ourselves and our loved ones?
It’s up to us as concerned citizens to ask these questions and get satisfactory answers before we agree to support the proposed health care reform bill. You can check out the website Free Republic, for a breakdown of some of the hidden points of the bill. If you’re like me, you’ll be shocked at how much government control and rationing there will be over every area of health care.
If these measures concern you then join other activists in your area to demand honest answers from your congress person.

I received your email newsletter today, with its bald-face lies about the healthcare proposals coming out of Washington. As if your personal, lying spin weren’t enough, you link us to Free Republic, which lists a litany of lies about the proposal that I myself have already checked out by reading and examining the plan.
You are a reprehensible example of what is profoundly wrong with the rabid Right wing of this country, and I will not have your language, your newsletter, your diatribes, your bullshit coming into my home via this email site.
We will have a public option healthcare plan, and you will learn to like it. In the meantime, I suppose I should expect that you, like so many liars on the Right, will begin to talk about President Obama and his plans for healthcare in Nazi terms. Will you call the plan a eugenics plan, as so many of your friends on the Right, including Glenn Beck, have done?

Apparently you haven’t spent much time in a physicians office lately.If any group is guilty of practicing ebm it is the doctors.It seems their main objective is to spend as little time with you as possible.I’ve been a registered nurse for almost 30 years and have watched doctors trend from caring professionals to overstressed businessmen.While I don’t support the healthcare bill or many of it’s components,I believe that a great majority of our populace would be better served by caring and dedicated nursing professionals.The majority of “care” is already delivered by them with physicians acting as phone consultants and then only due to legal considerations.

Our health care is worse than any other first world country, yet there are those who would rather that nothing change because of a perceived “government take over” of health care. All first world countries have a government option and amazingly enough, all of them have better health care than we do.
I normarlly read and enjoy the Total Health articles. But this twisted piece is completely without merit. Every person or organization that Melanie Sagala quotes is far off the chart to the right. I’m truly disappointed that this sort of politically-motivated trash can be printed in your newsletters.

I am an RN and fully resent your summations of the abilities of RN’s and PA’s, both of which would never attempt to administer the mentioned medicine without the oversight of a physician.

PermalinkIn my (fortunately, rather limited) experience with medical care it would appear that 90 odd, per cent of medical care is administered quite nicely by nurses, often as not while the docs are eithe on the golf course or rushing through their schedules to get there!
Ideally, we will have to ‘transition to wellness’ in one way or another. Your solution would be?
Thjis little idealogical hit piece is so distorted, so ‘political’ and so ‘WRONG’ that I am very disappointed that you’d attempt to make it look otherwise - to make it look objective and pro-wellness.

Tactics that have been used across our country to disrupt public meetings and conversations. It’s a sad shame when a group of people feel the need to use fear in an attempt to control people and outcomes. I would hope that humanity would be a bit more evolved!
In Oregon, we have what are called adult foster homes. They are small, licensed homes for 5-8 people who need on-going care and medical supervision. Typically, each home is operated by a person who lives on-site with their family. Medical care is still given off-site by physicians. There are several “levels” of homes - for those who are still able to conduct many of their activities of daily living but need supervision and stability, to those who need extensive care (such as Alzheimers patients) and everything in between.
My mother is living in an adult foster home. While her needs aren’t huge, they are still too much for me to handle because I have disabilities and a chronic illness. With a limited income, I would never be able to afford other options!
She has her own room and is surrounded by her own things. She has privacy and her own bathroom; regular, balanced meals; she is still independent enough to take day trips, to shop, to volunteer on occasion; and is free enough to spend time with and visit family as often as she wants.
Prior to her placement she was hospitalized several times near death. Since her placement, she is much healthier and still active and involved with her family and community.
A friend’s father is living in another home, one specifically for people suffering with Alzheimers. Another friend has a young adult son placed in an AFH that specializes in young people with severe developmental disabilities. What other options would be available? Institutionalization, with it’s higher cost and dehumanization?
Adult foster homes are a far cry from nursing homes and allow some dignity and independence. It gives our elders and people with extreme disabilities an option that includes quality of life, and it’s available even to those of low-income.
Perhaps some of these fear-mongers should do a bit of research and use their brain cells for something other than filling empty space and leeching light out of the world!

This article is biased, give only half the story and supports big pharma and insurance companies who are the only benefactors of half changes to our health care system. Cost savings would be easily attainable with state of the art management of existing systems and compatible medical records systems across providers. Medical errors and duplication of identical tests would be reduced. Improvements in care given could be realized. Impossible when your only motive is to put more money in the corporations profits…..Perhaps the real problem is the shallow self serving, short term mission statements of today’s corporations.

PermalinkAs a physician assistant, I was totally insulted by this article that implied that NPs and PAs are “partially trained medical personnel.” Both NPs and PAs go through intensive training to receive proper certification and licensing. Not only that, but PAs work in partnership with supervising physicians, so “unsupervised” is misleading.
Get your facts straight before maligning health care personnel who often function as patient advocates and cost-effective providers of quality healthcare.

I would venture to guess that progressives are much less of a burden on the healthcare system than Free Republic followers. Also, progressives are not scared witless by the likes of Glenn Beck/Hannity/Limpbaw. The screamers are people who need healthcare the most given the looks of them. I just don’t see where having a government healthcare OPTION in case you lose your job and forcing the insurance companies to stop throwing patients with pre-existing conditions off their rolls is a scary thing. I love when these people use the Post Office as an example of government ineptitude; 44 cents to mail a letter and it gets to the other coast in a few days; abolish it and watch the rates for UPS/FedEx go sky high.

It also is focused on already manifested disease to be “cured” by pharmaceuticals that create their own problems or surgery which has been shown ineffective, costly, and unnecessarily invasive. We need to prevent illness by protecting our food system, currently under attack by legislation favoring the industrial food system and undermining small, sustainable farms. Healthy produce, humanely-raised meats, and remedies from nature will do more to improve our health than any doctor. Remember, doctors are taught only to react to symptoms, and then only with drugs, surgery, or other interventions that never take into consideration the cause of the illness or the wholeness of the patient.

Entered: August 19th, 2009 at 10:40 am. PermalinkI am not a US citizen but I just can’t help make a comment about your ongoing debate. if I live in a country that had its health system rate (above or below) Solvina I would be looking for some radical reforms.
Your comment that suggest elderly people are not well served by seeing nurse practitioners should be reexamined. I am sure you can agree that many elderly people need more than 7 to 8 minutes to really discuss their medical problems and learn how to take care of themselves. Unless you want to spend 90%of your GDP on medical services it cannot be provided by doctors. Nurse practitioners work closely under physicians and can provide a link to overall medical care and can take the time to really take to people.
I am not involved in the medical industry. Recently my doctor moved his practice to a regional health authority and I came across this service model for the first time. I didn’t initially like it but I found I got a lot more individual time and attention and have the option of discussing any problems with my doctor at any time.
If you can move from a sickness model to a wellness model you would truly lead the world in provision of health services - as opposed to having international comparison like infant mortality rate make the US rank with any third world country.
I usually look forward to reading your newsletters, but like the others who have so far posted their comments today, I’m shocked that THB would publish this right-wing, corporatist propaganda! Melanie Segala, you should be ashamed of yourself! I think you owe your readers an apology and an explanation. Do your homework Melanie! I won’t go so far as to unsubscribe - at least not yet. Have many years managing doctors offices, which included NP’s & PA’s.
Most, if not all states, require NP/PA’s to have a supervising physician. As the physician assumes some medical liability, this system works well for weeding out any less than stellar NP/PA’s. (Granted it’s only as good as the supervising physician). For that reason all of the NP/PA’s I’ve worked with provided as good, and in many cases, better care than the physician.
Medicare allowed, and may still, that a physician can bill for services provided at a Nursing Home, by his NP/PA if he is in the building. So, we already have elderly attended to by NP/PA’s. If Medicare didn’t allow this - then nursing home patients would have LESS care - there is only 24 hours in a day, most FP’s work 10-12 hours a day. (Specialists & doc’s who earn money from other sources, such as web sites, probably spend less hours in face-to-face patient encounters.)
And finally, if you would look at the care provided to patients in hospice, I doubt you would dare cast such negativity towards the providers of care - most of whom are NOT MD’s. In fact I would venture that the same people drawn to hospice would be drawn to these clinics.
So, please tell your sources to go skip rocks, or somethine else that they may be more qualified to do.

Although we need to do our homework and read the healthcare proposal carefully, I am positive what you describe is not the aim of the Obama administration. One IMPORTANT thing the writer does not seem to grasp is that even if what she states about medical homes is true, the medical personnel MUST provide adequate medical care, or RISK LOSING THEIR LICENSES. That means they need to call in a licensed Medical Doctor, if they have ANY doubt about a medical decision. Also, all medical professions have Codes of Ethics that MUST be followed. All of these codes include a statement that the PATIENT’S INTEREST comes first! If this was Nazi Germany, where medical professionals could be ordered by the government to do the will of the government, then the writer might have legitimate cause for concern. BUT THAT IS NOT THE USA today, as we all know. I am a medical professional and anyone in the medical professions knows that what I am saying is the truth. HOWEVER, the current situation with our healthcare system is that INSURANCE COMPANIES can and do deny care to patients, based on their LEGAL RIGHT to do so under a number of different laws. Not covering preexisting conditions is just one such “right” they frequently exercise. Also, placing caps on how much of your medical expenses they chose to cover, is another one. For instance, they can limit your reimbursements for medical expenses to a set amount over your lifetime. Once you reach that limit, YOU HAVE NO RIGHTS. You simply have to sell whatever you own to pay your medical bills, until you have practically nothing, then Medicaid will begin to pay. Look at the bankruptcy statistics and you will see that across our country people are being driven into POVERTY, as a result of being sick! When you are most vulnerable, in the throes of an often terminal illness, you have to worry about losing everything, worry about how your family will survive after you are gone. Is this the America we want to live in! I have not even mentioned having to PLEAD with insurance companies when they tell you they will NOT PAY for a TREATMENT THAT YOUR DOCTOR RECOMMENDS, because they want you to accept another, less costly treatment, which your doctor has already decided is not right for your specific circumstances. The truth is we have limited control over our medical treatment NOW! You may be happy with your current medical care, because you have not needed really expensive care. Hopefully you will not need it for a long time. But the longer we wait to reform our medical care system, the higher the costs and the worse your insurance coverage will be. This is a certainty, if you look at the population statistics on aging. If we leave the system in the hands of the insurance companies, and certain entrenched parts of the medical system, you will continue to have higher, and higher out-of-pocket expenses and LESS AND LESS CHOICE!

It is understandable that ETR is far more profitable now, with the change from being focused on offering good advice to being focused on selling product-product-product-product.
